What happens if I connect this 48V fan to a 12V or 24V supply?

It will not spin, or it will spin very slowly and unreliably. A 48V fan typically has a startup voltage around 24-30V. Below that, the motor driver IC does not have enough headroom to commutate reliably. Do not attempt to drive it with a boosted voltage unless the boost converter can supply the startup surge current (which can be 2-3x the rated running current for the first 100ms).

Can I leave the PWM wire disconnected?

Yes. On Sanyo Denki 4-wire fans, an unconnected PWM pin defaults to full speed. The fan will run at its maximum RPM whenever power is applied. If you want speed control, apply a 25kHz PWM signal (0-5V logic level) to the PWM wire -- 0% duty = full speed, 100% duty = minimum speed (typically around 30-50% of max RPM depending on the model).
